Plainfield Health Care Center is a 189 bed facility, certified by Medicare and Medicaid. Plainfield Health Care Center has served the Hendricks County for over twenty years. We offer quality health care services to those individuals requiring a higher level of care than is practical or available at home.  


Plainfield Health Care Center serves our residents’ needs through multiple levels of care including skilled and traditional care, as well as respite and hospice care.

Skilled Care – Our physician-directed skills health care services are designed to meet the needs of every resident. At Plainfield Health Care Center, those needs are addressed with physical, occupational, speech, and nutritional therapies combined with 24 hour nursing care.

Traditional Care - Our Facility provides consistent nursing care to residents who need support in their daily activities. Through our care, these residents are able to achieve and maintain their highest level of functioning possible.

Respite Care – Our Respite Care service is designed to give family members a much needed break from the demands of caring for the chronically or terminally ill at home. Whether it is for a weekend or a few weeks, Plainfield Health Care Center provided our patients with a comfortable, secure, and social environment. Additionally, our highly-trained staff is ready to meet any medical needs.

Caring Hands Unit – Alzheimer’s disease is progressive, degenerative disease that attacks the brain and causes impairment of memory, thought process, and behavior. We believe all residents regardless of any cognitive limitations are entitled to be treated with dignity and respect in a manner that emphasizes individual strengths and promotes self-esteem. We are proud of our 59 bed Dementia unit, as it has been a source of positive outcomes for our residents who reside there.

Our Caring Hands Unit offers:

  • Secure Environment Family Support Group Educational Information Activities Social Services Respite Care
  • Day Care Program (Monday-Friday; 4 hour minimum) 

In addition, we also offer a complete line of specialized rehabilitative services for people planning on returning to their homes. These services are provided by certified, licensed therapists employed by our facility.  Our rehabilitative services include:

Physical Therapy – Helps patients regain their strength and mobility, manage pain, or adapt to a permanent disability

Occupational Therapy – Helps patients regain as much independence as possible with visual perception training, upper extremity strengthening and coordination, and self-care skills.

Speech Therapy – Helps patients overcome communication conditions such as aphasia, swallowing difficulties, and voice disorders.

Medical Nutritional Therapy – Promotes a better diet and healthier lifestyle with personal counseling and nutritional guidance.


Our dietary department offers delicious, well-balanced meals to accommodate individual needs, tastes, and preferences. We have a registered dietician on staff, and provide personal assistance to those residents who need it. We also allow residents to eat in the privacy of their room if they prefer.  


Our Center and the expertly trained activity staff offer a seven day a week recreational activity program for our residents. This program provides the opportunity for our residents to remain involved and maintain a better quality of life.
